Phase 5 – Transition
During this phase training is completed, solution knowledge is transferred to the support of the organisations, and the new solution is moved into every day life within the clients organisation. A post production assessment is completed to close our all project activities and responsibilities. The day-to-day communications are transferred to the customer support group.
The project manager then compiles all project documentation and assembles it into a final project report. Once the client has reviewed the final project report, the project acceptance document is submitted for signature. Once signed, the project is now considered completed and the solution delivered.
The tasks and deliverables during the transition phase are:

Continue end user training
Compile final project report with solution details and IT configuration
Hold transition meeting with customer support and clients help desk
Hold post production assessment meeting with project team
Obtain final client approval for project closeout
Initiate end user support program
